Community nursing brings registered nursing care directly to NDIS participants who need ongoing clinical support, in their home, their SIL accommodation or their community. Unlike general support workers, our community nurses are qualified clinicians.

Our Community Nursing Services

Specialist nursing for complex health needs

Our community nursing team is trained and experienced in a comprehensive range of clinical supports, all delivered under the NDIS in Western Sydney and surrounding areas.

Ventilator and tracheostomy care

Management of ventilator-dependent participants and tracheostomy care: monitoring, suctioning, circuit changes and emergency response training for carers.

Complex bowel care

Clinically managed bowel care programs, including digital evacuation, suppository administration, enema management and ongoing assessment.

Urinary catheter management

Management of indwelling and suprapubic catheters: insertion, removal, routine maintenance, troubleshooting and participant and carer education.

Continence assessment

Thorough NDIS continence assessments with tailored care plans, product recommendations and support for self-management where possible.

Enteral feeding support

Management of PEG tubes and nasogastric feeds: feed administration, tube site care, troubleshooting and education for carers and family.

Medication assistance

NDIS-funded medication assistance with clinical oversight: administration of complex regimens, webster pack set-up and medication reconciliation.

Epilepsy and seizure support

Seizure monitoring, rescue medication administration, seizure diaries, first aid training for carers and emergency response planning.

Health assessments

Annual and acute health assessments by our registered nurses to identify needs, guide care plans and inform NDIS plan reviews and referrals.

Care plan development

Clear, evidence-based care plans for complex participants: clinical needs, goals, risk management and support worker instructions.

How to access community nursing through the NDIS

Setting up community nursing support is straightforward. Here is how we begin.

Enquire or refer

Call 1300 264 942, send us a message or submit a referral. We accept referrals from participants, family, support coordinators and hospital discharge teams.

Clinical assessment

Our team conducts an initial health assessment to understand the participant's clinical needs, risks and goals, then develops a tailored care plan.

Nursing begins

Your consistent community nursing team begins scheduled visits. We communicate openly with family, support workers and the broader care team throughout.

How is community nursing different from a support worker?

Support workers assist with daily tasks. Community nurses are qualified clinicians who deliver and oversee medical care. At 1 Care Connect the two work together: our in home care is nurse-overseen, so clinical and everyday support join up.
