Specialist NDIS community nursing delivered by qualified Registered Nurses: clinical expertise, compassionate care and consistency for complex health needs, in your own home or community.
Community nursing brings registered nursing care directly to NDIS participants who need ongoing clinical support, in their home, their SIL accommodation or their community. Unlike general support workers, our community nurses are qualified clinicians.
At 1 Care Connect, community nursing sits at the core of everything we do. Our nurse-led model means every participant, whether they receive in-home care, SIL or stand-alone community nursing, benefits from clinical expertise that shapes their whole plan.
Our community nursing team is trained and experienced in a comprehensive range of clinical supports, all delivered under the NDIS in Western Sydney and surrounding areas.
Management of ventilator-dependent participants and tracheostomy care: monitoring, suctioning, circuit changes and emergency response training for carers.
Clinically managed bowel care programs, including digital evacuation, suppository administration, enema management and ongoing assessment.
Management of indwelling and suprapubic catheters: insertion, removal, routine maintenance, troubleshooting and participant and carer education.
Thorough NDIS continence assessments with tailored care plans, product recommendations and support for self-management where possible.
Management of PEG tubes and nasogastric feeds: feed administration, tube site care, troubleshooting and education for carers and family.
NDIS-funded medication assistance with clinical oversight: administration of complex regimens, webster pack set-up and medication reconciliation.
Seizure monitoring, rescue medication administration, seizure diaries, first aid training for carers and emergency response planning.
Annual and acute health assessments by our registered nurses to identify needs, guide care plans and inform NDIS plan reviews and referrals.
Clear, evidence-based care plans for complex participants: clinical needs, goals, risk management and support worker instructions.
Education sessions for support workers, household members and family carers, building clinical confidence around the participant.
Liaising with GPs, hospital discharge planners, allied health and specialists so nothing falls through the gaps in the care team.
All community nursing is available in the participant's home, SIL accommodation or community setting, bringing nursing care to where it is needed.
1 Care Connect was founded with nursing at its core, not as an add-on but as the backbone of every support we deliver. Our community nursing team brings hospital-grade clinical standards into participants' homes and communities.
Participants do not just receive care. They receive clinically overseen, evidence-based support from nurses who know their history, understand their goals, and actively coordinate with their broader medical team.
Our community nursing services are designed for NDIS participants whose clinical health needs go beyond what a typical support worker can safely manage. If you or someone you support has complex health requirements, our community nurses can help.
Community nursing is funded under the NDIS as a Capacity Building support under Health and Wellbeing. Speak to your support coordinator or Local Area Coordinator to include it in your NDIS plan.
Clinical thinking shapes every service we deliver, for every participant. That is what hospital-grade care at home really means.
Clinical skill is essential in community nursing. But so is how you show up: with humanity, consistency and genuine care for the person in front of you.
Grounded in Christian values: compassion, integrity and a deep respect for every person's dignity, whatever the complexity of their health needs.
Participants are not handed between different nurses each visit. You have a consistent clinical team who knows your health history, preferences and goals.
We are not just a provider that employs community nurses. Clinical thinking shapes every service we deliver, for every participant.
